MAKEUP LESSON REQUEST FORM

Enrolled LTM students are permitted 1 makeup lesson a month with 24-hour notice prior to your students scheduled lesson time. Please fill out this form listing the date you will be missing & why, while also including your availability for makeup scheduling in the next 2 weeks! Please note, if you have already redeemed your 1 makeup lesson for the month - we will not be able to reschedule. All makeups will be redeemed in-studio or online. Thank you!
